A program developed to improve outcomes for patients with Chronic Obstructive Pulmonary Disease (COPD) by providing education in print and verbally on their disease process, the lifestyle changes and medications to manage their disease, and how to prevent acute exacerbations that can lead to hospitalizations. Development and implementation is by a multidisciplinary team including nurses, therapists and telehealth home telemonitoring which is a cornerstone of the program.
